Dear Swimmer & Parents

Welcome to Gallica and good luck with your 2 week trial.

Please complete the form below with the necessary details and send to Gallica admin as soon as possible, by clicking the submit icon at the bottom of this form.

If on completion of the trial period you are accepted and after talks with your squad coach you make the decision to stay and become a member of Gallica, you will be asked to complete a Membership Welcome pack with all the relevant details and documentation you will need to complete in order to become a Squad Member. You will however always remain a member of your 1st claim club.

Membership fees will fall due on the first day following the 2 week trial and the initial amount payable will be calculated pro-rata for this period.

Your monthly fees will then be due and paid by Standing Order on the 1st day of every month into the Gallica (Lancashire) S.C. account. A Standing order document will be enclosed in your pack.

Your Annual ASA registration fees will still remain payable through your 1st claim club. The Gallica Annual Club Membership fee falls due at the beginning of October each year. This is currently £10.00.

DISCLAIMER

It is a necessary requirement for you to complete and return this form before commencing your trial at Gallica. If we have not received this form before your trial period has started, you will be unable to train with the squad.

If your trial is unsuccessful, we will retain your details on file, should you wish to return at a later date. Should you decide not to take this option your details will be deleted.
